  4. Sedula (Individual/Corporation)
  5. RPT Payment (Amilyar) Clearance
  6. DTI/SEC Registration
  7. Health Certificate of all food handlers
  8. DOLE accreditation (for Manpower Agency)
  9. For lessee (Rental Permit from Lessor) 

BUSINESS RETIREMENT

  1. Affidafvit of retirement/ closure of Business ( End of operation and reason)
  2. Latest Mayors Permit and official receipts
  3. Barangay clearance – retirement
  4. Gross Sales/ Receipt of last/ current year of operation
